Filing Deadline

Processing Time

The encounter itself occurs during a clinical visit. Documentation should be provided to the home health agency within 5-10 business days.

What Happens Next

The home health agency includes the face-to-face documentation with the CMS-485 plan of care. Without valid face-to-face documentation, Medicare will not pay for home health services.

Tips for This Form

  • The encounter can be a telehealth visit in many cases — CMS expanded telehealth eligibility for face-to-face encounters
  • The documentation must specifically address homebound status AND need for skilled services — both are required
  • A hospital discharge does not automatically satisfy the face-to-face requirement unless the documentation explicitly addresses both elements
  • NPs, PAs, and CNSs can perform the encounter but the certifying physician must still sign the CMS-485
